Milestones and change requests

Questions
When should I hand over the code?
Many developers transfer source code and admin access when the final invoice is paid. If that is your practice, say so in the terms so it is agreed in writing.
Can I invoice a retainer each month?
Yes. Use one line for the monthly retainer and extra hourly lines for work beyond it. Use "Next number" each month to keep the numbering in sequence.
Do I charge VAT or GST to overseas clients?
Rules for cross-border digital services depend on your country and the client. Set the rate you need, or zero with a note such as "export of services" where that applies, after checking the rule.
