What makes it usable as a superbill

Insurers generally ask for the provider name, credentials and NPI, the client name, the date of each session, the service code, the fee and the amount paid. Diagnosis codes are usually needed too, which is why the field is there but optional.

Questions

Which service codes do therapists use?

In the US, sessions are usually billed with CPT codes chosen by session length and type. Enter the code you use in the service code column; the tool does not choose codes for you.

Should therapy be taxed?

Therapy by a licensed practitioner is usually exempt from sales tax, VAT or GST, which is why sample lines are marked as not taxable. Check the rules for your profession and country.

Can I show a sliding-scale fee?
